The 12 words you get from the vault are different than the 12 words used by Counterparty. Counterparty implemented the mnemonic phrase before Bitcoin, so they don't jive. You need to go into freewallet, click change address on the little qr code icon top right, then click options where it allows you to import a private key

You paste that private key in, and you should have access to your card. You'll need to send BTC to that address to be able to move the card out, then send the remainder of your BTC out too. Sometimes that can get hung up using freewallet, so you'll need to use electrum with that private key if that's the case.
